How accurate is it to make a guide at the time of extraction vs the time of placement (which may months or years later)?

The safest time and manner to place place an implant without a CT scan is when you can see directly into the root socket at the time of extraction. Safegide allows dentists to make a guide while they can see into the socket, then place an implant into the precise location of the original socket even after the socket has healed.

There are multiple factors that ensure accuracy of Safegide surgical guides.

  1. we always recommend filling the socket with bone and retaining the adjacent teeth in position
  2. The Safegide jigs (the part that houses our stainless steel drill sleeves) are made from stable, distortion free composite. The jigs do not warp or shrink.
  3. Safegide's accuracy can be verified using the Verification points that come with the kit. These points indicate the intended path of the implant drills before the procedure is started.

How can an implant be safely positioned using a guide made in only a few minutes by a dental assistant?

 Most guides are completed in 5 minutes or less.  The two primary reasons for these short treatment times are:

  1. The assistant takes full arch impressions, then casts the models used to make a  composite jig that will  house the drill guides. This is usually done at the appointment BEFORE extractions.
  2. The splint is checked before extractions. Once the tooth is out, it takes a few seconds to position the insert and bond the metal sleeve on the insert to the splint.

How do I know which implants I should avoid?

Time and risk!

We believe that general dentists should refer out the grafting part of implant procedures only (if they are not comfortable doing these grafting procedures). Implants are easier if you have adequate tissue and you have an accurate guide.We also believe that you should not do any procedure that you are not comfortable doing.

Many elderly patients that are missing teeth are also high risk. Obviously we should not treat high risk patients without medical clearance.   3. A composite jig (it looks like a bite-splint with a slot above the tooth to be extracted) is then made on the pre-extraction model. A space is made above the tooth to be removed. The jig may be stored until the patient returns for the tooth extraction. 
  4. Immediately after extraction the jig is positioned in the mouth and the selected composite Safegide®socket insert is placed through the jig into the socket.The selection of this insert is determined by the anatomical structures at the intended site. The socket inserts are easily adapted to the lengths of the sockets.
  5. The stainless steel guide-sleeve on the insert in the socket is bonded to the pre-fabricated jig.
  6. Verify accuracy on X-ray using the verifier points before drilling the osteotomy.The information provided by the assembly will be used to determine the safest osteotomy position, angulation and length of the implants to replace the missing tooth, as well as the ideal abutment position. The process cannot be easier or more accurate.

The guide can be used to stabilize the drills during the exodontia visit or after the bone has healed.  The drill guide will focus and aim the pilot drill preventing unwanted movement, slipping or skipping. Drilling into a specific target in the socket like the septal bone, or, at an angle into the lingual bony wall is predictable and easy.

All Safegide components are easily visible on X-ray so drill stop and implant length calculations are easy. 

The guide should also be retained for accurate exposure of any healing caps that have been covered with soft tissue.

The universal reduction sleeve tool allows the same sized guide-sleeves to be used for multiple drill diameters ranging from 2-6mm. The instrument assists in holding the tongue and cheek out of the way during the pilot drilling procedure. This nifty instrument is found only in the Safegide® kit.

How is Safegide® different to traditional guides?

Safegide® provides a guide that is more accurate because it is formed in the patients mouth. It is checked for accuracy as well as intended occlusion during fabrication, but at a fraction of the cost and taking a fraction of the time of any other similar product. This will allow more dentists to place implants safely, and, more patients to afford implants.

Traditional implant placement, as most often performed by an oral surgeon or general dentist seeks to achieve perfect positioning using indirect vision. This is typically done using a CT scan of the patients healed socket, and sending the CT scan to a specialist lab where a CAD guide is fabricated. This usually involves time, inconvenience and great expense. These place barriers in front of implant affordability.

Other less expensive guides rely on guesswork and using clear matrix vacuum templates. These are too soft and inaccurate.

Is a Safegide® Dental Implant Guide Kit for everybody? What if I only restore implants.Should I still use the Safegide® Dental Implant Guide Kits?

This system is perfect for the dentists who do not place implants because of the financial and malpractice risks associated with inadequate education, equipment and poor placement. These dentists may also be referring out simple extractions because they do not want to interfere with the implant placement sequence. They also hope (keep dreaming) that whoever places the implants will refer the patients back so that they can earn some income from placing the crown on their patient’s implant. This may not happen as more and more implant clinics hire their own dentists. You can kiss your patients goodbye when you make this referral.

The Safegide® system changes everything. You can still elect not to do the implant but participate in the other most lucrative steps (the extraction, bone graft, guide fabrication, implant restoration).

Is inaccurate implant placement a concern with Safegide®?

Inaccurate implant placement is not of more concern with the Safegide® than with any other type of implant osteotomy drill guide including those made with a CBCT scan. In fact, there is less chance for inaccurate placement using the Safegide® because the guide aims the drill into the healed socket site of the missing teeth.

Safegide® allows pre-op verification at the time of extraction, and, any time in the future prior to using the pilot drill. The intuitive selection guide ensures that the dentist correctly chooses the ideal implant position as well as the best abutment angle and locationfor optimal occlusal function. Lab fabricated guides and implant placement prescribed by the oral surgeon often fail to take ideal occlusion into consideration. The Safegide® allows selection of implant location as well as abutment positions.

Is this similar to other chair-side or lab fabricated guides? Why is it faster?

There are many important differences between the Safegide® and those inherent with other chair side or lab fabricated guides.

  1. Treatment Time – Other guides are usually made in days or weeks by a commercial lab. With the rapid fabrication system, most of the work is done in your office before the extraction appointment. The guide is completed in minutes at the time of extraction while the patient is numb.
  2. The Safegide® is also verified as being spatially accurate in three dimensions. Most clear matrix lab positioned guides may position the entry point of the exploratory drill in the ideal position but fail to provide accurate angulations and depth.
  3. Safegide® will accommodate and center all drill diameters from 2mm to 5.5mm. Most lab guides are restricted to smaller drill sizes.
  4. Osteotomy guides made with Safegide® Dental Implant Guide Kits can be completed in minutes saving the patient time, inconvenience, money and post-op problems.
  5. Predictable accuracy – Lab fabricated guides can be limited in achieving desired implant positions. With the Safegide®, there is better accuracy in a wider range of potential placement sites, as well as more options to avoid unexpected vital structures. Should dentists still need to buy a CBCT scan device for implant placement?

Depends. If you can justify the expense of a CT scanner and you have the space in your office.  However if the CT scanner is going to stress your finances because it will not improve your profit or productivity then you should think it through carefully. Safegide kits are low cost disposables that will allow dentists to get started on safe implant placement without any financial risk.

Fortunately Safegide® Dental Implant Guide Kits allow most implant osteotomies to be done using accurate guides made without CT scans. The Safegide® kit is effective for almost every osteotomy. The system works best at the time of extraction. 

The main reasons for taking the CT scans are to show you what you cannot see with the naked eye after the exodontia site has healed. This includes osteotomy information on where to place an implant. The Safegide® system makes it easy for you to record all this information when you can see into the socket at the time of extraction.

We recommend that you always make a guide at the time of extraction.When you are ready to do the procedure you may verify the accuracy of your Safegide guides with a CBCT scanner. This is especially useful when the case does not allow additional time to wait for a laboratory fabricated guide.   

Safegide implant drill guides should not be used if the dentist is unsure of there being sufficient bone or the proximity of sensitive anatomical structures, like the mental nerve or the lining of the nasal sinuses.
